Detect duplicate and near-duplicate images across hospitals

Compute perceptual hashes for every image in the frozen splits and flag exact and near-duplicate pairs, especially pairs that cross the training and held-out hospital boundary. Deliverable: A duplicate report with image_a, image_b, hash_distance, and split membership, plus the rule used to remove or keep each pair.
